This allows me to get to know you and your hair goals so that we can ensure your initial appointment is booked properly and I can give you the quality of service that you deserve! Please complete the form in full and book your appointment titled NEW GUEST [service] through "RESERVE NOW" on the services and booking page. All appointments will be looked over with this form and the timing will be adjusted accordingly to your answers on this page. The Beauty Project respects everyone’s choice to wear a mask or not while in the salon as long as we are not in a mask mandate. I just ask that if you are sick, please reschedule your appointment. If you are within the 48 hour cancellation period and you are feeling sick AT ALL. Email me right away and we will come up with a game plan to reschedule your appointment. I don’t want to get sick and I don’t want to get others sick. This goes beyond Covid and includes colds and flus. If you come in sick, I will ask you to leave and we will have to reschedule. By completing this form, you are submitting along side with booking your own appointment in which timing will be updated by The Beauty Project once looking over your consultation form. Your appointment in salon will begin with a 30 minute complimentary consultation to refine the plan for service.

You understand that The Beauty Project has a strict 48 hour cancellation policy and if you do cancel or reschedule your appointment with less than 48 hours notice, a 50% charge of the service missed will be added to your profile and must be paid before you book your next appointment.

if you are running late please be sure to contact me. If you are more than 15 minutes late we may have to reschedule your appointment for another time and he will be charged according to the original appointment time.
