This form is used to request access to the internal Ayzenberg Group network from an external computing device. A Virtual Private Network (VPN) Account Sponsor, i.e. an Ayzenberg Group department lead, is responsible for filling out this form in order to grant employee access to the Ayzenberg network. Please note that improperly completed forms will delay processing of your request. As the VPN Account Sponsor, you are responsible for ensuring that network access privileges granted by this VPN account are used in accordance with the Ayzenberg Group Data and Security Policy. *
