Use this form to submit your voluntary 30 day notice to vacate to indicate you and all named renters/occupants will be vacating the unit/home.
This 30 day notice to vacate must be provided a full 30 days prior to move out. Your are responsible for paying the rent through your 30 day notice period even if you vacate the unit/home earlier.

**IMPORTANT** You may NOT use this notice to vacate form if the following applies:
- You want to vacate/break your lease before the end of your lease term
- You are a roommate and want to move out

If you have chosen to receive your move out disposition statement by EMAIL and security deposit refund, if any, by DIRECT DEPOSIT straight to your and all named renters bank accounts, you agree that ALL named renters on the lease must separately complete the Electronic Security Deposit Accounting and Refund Agreement. The security deposit refund, if any, will be split evenly among all named renters on the lease. You also agree to pay a $50.00 administrative fee for electronic deposit of your security deposit refund (if any) and agree to have this amount deducted from your security deposit (if any). Should all named renters on the lease fail to separately complete the Electronic Security Deposit Accounting and Refund Agreement, the move out disposition statement and security deposit refund, if any, will be sent via regular mail to the new forwarding address provided above. After submitting this form, you will see a confirmation screen with the a link to complete the Electronic Security Deposit Accounting and Refund Agreement. You will also receive an email confirmation with the link to the email address you provided above.*

If you have chosen to have your move out disposition statement and security deposit refund (if any) mailed to you, your security deposit disposition statement will be mailed to your new forwarding address (pursuant to California civil code §1950.5) within twenty one (21) calendar days of your move out. The security deposit refund (if any) will be made payable to ALL named renters on the lease. If you do not provide a new forwarding address, your security deposit disposition statement and security deposit refund, if any, will be mailed to your last known address. Unpaid rent (which includes late fees and other outstanding charges) 2.
