Report Form

A Pastoral Support Committee (PSC) provides their Commissioned Minister with ongoing guidance, support, and encouragement as they serve in ministry. It is recommended that the PSC meet every once every one to three months to engage in meaningful conversation with their Commissioned Minister. This PSC Report Form documents each Meeting held between a Commissioned Minister and their PSC.

Please note that the Reports for each PSC Meeting will be stored in the Commissioned Minsiter's clergy file to be read and reviewed by their Commissioned Ministry Team (CMT).
