A Commissioned Minister's Pastoral Support Committee (PSC) provides ongoing guidance, support, and encouragement for their service in ministry. It is recommended that the PSC meet every one to three months to engage in meaningful conversation with their Commissioned Minister.

This PSC Report Form documents a Meeting or Conversation held between a Commissioned Minister and their PSC.

PSC Meeting Report Forms are stored in the Commissioned Minsiter's clergy file to be read and reviewed by their Regional Commissioned Ministry Team (CMT).
