    This form is to be used for Kindergarten registration and students new to MBA in grades 1-9
  • Before you begin

    Note: Please read the following instructions carefully before beginning.

    In order to complete this document you will need:

    1. A picture / PDF of your child's Birth certificate, Permanent Resident Card (if applicable), or other Work Visa Documents (if applicable).
    2. A picture / PDF of your child's most recent report card (if they have attended school previously).
    3. Your child's Alberta Health Number
    4. Your child's Alberta Student Number (ASN), if they have one.
    5. A picture of your child (shoulders up, taken on your phone works great)
    6. A picture of mom and dad (individual pictures, shoulders up, taken on your phone work great)

    If you are unable to complete a section of this form because of lost documentation, please contact MBA at 780 619 8882.

  • Why does MBA want parent pictures? Good question. As a result of COVID-19 (ie. less face to face interaction),  and as the school grows in size, MBA staff want to do a better job of putting a face to the name of our MBA parents. 

    What type of picture should I submit? Just a picture from the shoulders up (snapped on your cell phone) is fine!

  • Human Sexuality Exemption Notice

    The following subjects in the Alberta program of studies cover subject material that deal with human sexuality: 

    • Health (Grades 4-6)
    • Health and Life Skills (Grades 7-9) 

    As per section 58.1 of the Education Act parents have the right to request (in writing) that their children be exempted from such instruction. For the purposes of this form, checking the box below, and signing the attestation at the end of this registration form shall constitute your request for your child to be exempt from content in the above mentioned classes that deal with human sexuality.

    Internal Use

    I agree and give my permission for Meadows Baptist Academy, to record (audio/video), copy, film or photograph my child’s name, image, student work, and performance (hereinafter collectively referred to as “Works”) and to distribute these Works for the purpose of publishing, posting on school websites, posting in schools, posting on social media sites and/or for broadcasting on television or radio as determined by Meadows Baptist Academy (MBA).

    I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to claim royalties from Meadows Baptist Academy related to the use of these Works. I understand that the Works may appear in electronic form on the Internet or in other publications outside of MBA’s control. I agree that I will not hold MBA responsible for such unauthorized reproduction.

    External Use

    I also understand that external media organizations may attend MBA events. I give permission for my child’s name, image, student work, and performance to be recorded (audio/video), copied, filmed or photographed for the purpose of being published and/or broadcast on-line, on television or radio.

  • Statement of Commitment 2022-2023

    In making application to Meadows Baptist Academy, I understand I am partnering with this educational institution for the spiritual, academic, and character development of my child. I understand it is the parent’s responsibility before God to accomplish these tasks (Eph. 6:4), but I am enlisting Meadows Baptist Academy to assist me in that responsibility. Furthermore, I understand that it is my responsibility as a parent to speak respectfully of the staff at MBA. I acknowledge that a positive relationship between parents and MBA is essential for the my child to be successful.

    ACADEMIC GUIDELINES

    • I understand the administration of Meadows Baptist Academy wants my child to receive the best possible education, and I will allow them to place my child at the grade level most beneficial to them. Also, I agree to allow the school to access RCSD (occupational therapist and speech language pathologist) should my child need extra help in their academics.
    • I understand that, in order for my child to derive the maximum benefit from his education, I may need to work with them at home if they experience difficulties in mastering material.
    • I understand that regular class attendance is essential for academic progress and is therefore required. I understand that, should my child miss significant class time, I as the parent, bear the primary responsibility for catching my child up.

    DISCIPLINE GUIDELINES

    • I understand that discipline, at times, is necessary in developing Christ like character in the life of a Christian young person (II Timothy 3:17) and give my permission to the teachers and administration to implement the needed discipline as prescribed in the Parent/Student Handbook. I understand this could include detention, suspension, and expulsion or a variety of other service projects.
    • I also understand that in developing Christ like character in our young people, it is our goal that our child(ren) exhibit Christ like characteristics both in and out of school. With that in mind, I understand that Meadows Baptist Academy is asking their students to not participate in those activities that do not honor God, whether at home or at school. MBA exists to collaborate with parents, not in order to replace them. For the partnership between home and school to be effective, the expectations must be the same at home as they are at school.
    • As a parent, I agree to support Meadows Baptist Academy with a cooperative attitude and a positive spirit. I understand that a critical spirit (Philippians 4:8) is unbiblical and agree to follow the outline that is exhibited in Matthew 18 should I disagree with the actions of the faculty or with school policies.

    FINANCIAL GUIDELINES

    I understand that it is necessary for tuition and fees to be paid on a timely basis for Meadows Baptist Academy to meet its financial obligations. I agree to pay any fees incurred as a result of late tuition payment. I understand that, in the event the tuition and fees are not paid on a consistent basis, my child may be denied re-enrollment at MBA.

    1. Tuition for the 2022-2023 school year is as follows:
      1. Kindergarten: no tuition
      2. First child: $150/month for 10 months
      3. Second child: $140/month for 10 months
      4. Third child: $130/month for 10 months
      5. Each additional child:  $120/month for 10 months

    2. Registration Fees for the 2022-2023 school year are as follows:
      1. Grade 1-3 new enrollment registration fee: $450
      2. Grade 4-9 new enrollment registration fee: $465
      3. Kindergarten registration fee of $1200
        1. $600 due upon registration
        2. $600 due January 1

    3. Re-enrollment fees for the 2022-2023 school year are as follows:
      1. G1-3 early re-enrollment fee: $400
      2. G4-9 early re-enrollment fee: $415
      3. G1-3 regular re-enrollment fee: $450
      4. G4-9 reular re-enrollment fee: $465

    4. Other Financial Items.
      I understand that:
      1. There is a mandatory $250 fundraising amount / family.
        1. Should I not raise the mandatory $250 fundraising amount I will be responsible to make up the difference between whatever I have raised and the mandatory $250.
      2. There is a technology fee of $150/student (grades 6-9), payable September 1.
      3. School supplies are provided at the cost of the parent
      4. I may incur some additional fees for school field trips.
      5. Registration fees (if the child is successfully enrolled) are non-refundable.
      6. Tuition paid after the 10th of the month will result in a $50 late fee.

    GENERAL GUIDELINES

    • I will not hold the school or its personnel ultimately responsible for any injury sustained at school, at a school-sponsored activity, or during athletic activities.
    • I will endeavor to drop off and pick up my children according to the times laid out in the parent/student handbook.
    • I recognize that it is my responsibility to contact the school should any of my contact information, children's medical information, or other information need updating.
    • If, for some reason, I am not receiving school emails, I will contact the school immediately.
    • I understand that, in the case of an accident or serious illness, parents will be called to determine the course of action. In case of a serious accident or life-threatening situation, both the parents and an emergency medical service will be contacted. I will notify the school immediately of any changes on the contact names or phone numbers listed on the emergency sheet so that the school will have current contact information and be able to reach someone quickly in an emergency.
    • I recognize that Meadows Baptist Academy is a distinctly Christian school, and as such, I agree to support the teachings of the school’s statement of faith as outlined in the Parent -Student Handbook.

  • Attestation: I, either the above mentioned father, mother, or guardian of the child listed above, acknowledge that the information contained in the above form is true and accurate to the best of my knowledge. Furthermore, I acknowledge that I have read and agree with the 2022-2023 parent-student handbook and the 2022-2023 statement of commitment. I also acknowledge that, should any of the above information change, it is my responsibility to notify the school.
