In consideration of being permitted to participate in the Bloomfield Chamber of Commerce Christmas Around the Courthouse event, I, for myself and on behalf of my heirs, executors, administrators and assigns, personal representatives, and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE BLOOMFIELD CHAMBER OF COMMERCE AND THE CITY OF BLOOMFIELD and its agents, employees and volunteers from any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of THE BLOOMFIELD CHAMBER OF COMMERCE AND THE CITY OF BLOOMFIELD, or otherwise, to the fullest extent permitted by law, and I hereby waive, release and forever dischargeTHE BLOOMFIELD CHAMBER OF COMMERCE AND THE CITY OF BLOOMFIELD from and against any and all claims, demands, actions, causes of action, and for costs, loss, expenses, or damages to personal property or personal injury or death which may result from my participation in this event. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I hereby certify that I have read, understand, and agree to the above statements. Further, by signing below, I certify that I have received a copy of the Rules and Regulations for All Vendors and agree to abide by the terms thereof.
