I understand that booking is limited and based on my available dates and/or design I may not be selected for a tattoo appointment.
I understand the studio will contact me by email or text ONLY IF my booking form is selected to continue the booking process.
I agree that myself and anyone else inquiring on the booking form is over the age of 18 or will be 18 by the time of the appointment. I acknowledge that Texas does not grant parental consent for tattoos to anyone under the age of 18.
