LEASES ARE SUBJECT TO THE BOARD OF MANAGERS’ EXERCISE OF ITS “RIGHT OF FIRST REFUSAL” AND MUST BE IN COMPLIANCE WITH ALL PROVISIONS & ARTICLES OF THE CONDOMINIUM'S OFFERING PLAN, BY-LAWS & HOUSE RULES
The below submittal must be completed and all required information and documentation included prior to submittal. This form can be submitted by prospective renter, owner, broker or representative. There is no fee associated with submitting this form.
Additionally, each lease applicant listed on the lease must complete the Kaito Authorization for Background Check and Payment form.
Once fully submitted, Kaito will present a completed package to the Board of the 198 21st Street Condominium. Applicants are not permitted to move into the unit until they have received Board waiver of the right of first refusal and until they have scheduled their move-in with building Management.
Please allow approximately 1-2 weeks for processing.

There is an application fee of $150 per person, payable by credit card, payable on the Authorization for Background Check and Payment form
Applicant must notify management at least 72 business hours prior to move and must provide required proof of insurance for movers and moving deposit, as detailed on the

If broker(s) are representing the prospective tenants(s) or owner on this sublet package, then all documents for this submittal should be collected, reviewed and submitted by broker using this form. Each prospective purchaser should only complete the Authorization for Background Check and Payment form.
