This release is made in favor of the UFW Foundation and their successors, assignees, or affiliates (hereinafter “Releasee”), by*
Name of the featured person
(hereinafter “Releasor”) regarding an appearance in photograph, film, video and audio tape, Internet, written matter and regarding any statements, declarations or other written matters by Releasor.

Releasor hereby acknowledges that it understands and certifies that it knowingly and voluntarily grants to Releasee, its agents, employees and assigns the right to record and edit into photograph, film, video, audio tape, Internet, and into written form Releasor’s name, image, likeness, voice and written statements as Releasee sees fit to do so.

Releasor acknowledges that Releasee shall be sole and exclusive owner of such photograph, film, video, audio tape, Internet, and written statements possessing the name, image, likeness, voice, and written statement of Releasor and shall have the exclusive right in perpetuity to use, exploit, distribute, and publish in any manner and in any and all media throughout the world, in all languages, as Releasee in its sole and unfettered discretion may determine. This release is governed in accordance with the laws of the State of California.

El Cedente reconoce que el Beneficiario será el dueño único y exclusivo de toda fotograf ías, película, cinta de video y audio, Materiales Internet, documentos escritos que poseen el nombre, la imagen, la semejanza, la voz, declaraciones por escrito del Cedente; y el Beneficiario tendrá el derecho exclusivo en perpetuidad para usar, explotar, distribuir de cualquier manera, o por medios de comunicación a través del mundo, en cualquier idioma, y en la forma que lo determine a su total discreción. Este formulario de divulgación legal se rige de acuerdo con las leyes del estado de California.
