This authorization and consent for release of my personal information acknowledges that the Downtown Hope Center, DHC, and its agent Alaska Christian Ministries Assoc, may now or at any time I am assigned to volunteer at DHC conduct investigations into criminal records, dept. of correction records and sex offender records.

I understand that these records will be used to determine my eligibility for volunteer duties at DHC. Therefore, I authorize and consent to the full release of those records to the authorized representative of DHC; and I release the custodians of the records from all liability to me that might arise from retrieving and accurately reporting the information to DHC.

I/we know and acknowledge that persons served by DHC are, in the nature of DHC’s mission, sometimes ill, unstable, abusive and/or violent, and that assisting its clientele can entail risk to myself of possible bodily injury, property damages, illness and /or other loss. I willingly assume that risk. If my volunteered service is accepted by DHC, I fully and completely release DHC from any and all liability the law might otherwise impose on it for any such injury, damage, loss or other harm to myself and /or personal property that might arise or occur in the course of my service.

Child Policy

We welcome you and your child(ren) to volunteer together. Children ages 0-5, must have an adult volunteering with them per each child. Children ages 5-14, must have an adult volunteering with them per three children. Children 15 to 18 years old can volunteer without adult supervision. Parents/guardians are responsible for their children. The Downtown Hope Center is not responsible for caring for your child while you are serving. If your family is deemed a safety hazard by the Downtown Hope Center staff, you may be relieved of your volunteer duties.

Example: If you are volunteering with five minors under the age of 14 there should be two adult volunteer applications with the minor information portion of the application filled out.

This is a safety measure for both you and your family, our staff and our building.
