I understand by signing this form it is not guaranteed that Happy Friends Rescue will be able to take in this pet. The rescue can contact you via email at any time to get more information about the pet if needed. If the rescue is able to take in the pet, a copy of the vet records must be sent before pick up. A service fee of $50 is required for surrendered animals.
