1. Complete your first exam with a doctor at WLVC. (We will reach out immediately after you submit this form with dates and times).
2. Complete pre-surgical bloodwork for your pet. We can gather a sample during your first visit.
3. Book your procedure with one of our doctors within 60 days of your bloodwork.
