PROJECT GRADUATION

Senior Contract

		$80 - February 14th - through June 3rd
		$100 - June 4th - Cash Only at the Door

*Scholarship grants are available but limited - Requests must be submitted to your Counselor*

Acknowledgment and Waiver

In order to attend the WHS Project Graduation Party, both a Parent/Guardian and the Senior must acknowledge and agree to the following:

All WUSD seniors are invited. Graduates will be asked to show proof of identification.
The party begins at 9:30 pm and ends at 3:30 am. We suggest making arrangements to drop off and pick up your Senior as they will be extremely tired.
Parents/Guardians of seniors who have not checked in by 11:00 pm will be notified their student has not arrived.
Lockers will be provided for storing belongings.
The WHS Project Graduation committee has no control over student postings of Project Grad activities on social media.
Photographs and/or video may be taken or recorded to be used by the WHS Project Graduation committee and/or Windsor High School for promotional purposes.

If a Senior chooses to leave the party early:

a. Parents/guardians will be contacted prior to allowing them to leave.

b. Seniors leaving the party before it ends will not be re-admitted.

c. Seniors leaving the party before it ends will forfeit all prizes.

Procedures

Under no circumstances will any senior who is under the influence and/or is in possession of drugs and/or alcohol be admitted to the party. Personal items may be searched on entry. Windsor High School PROJECT GRADUATION committee will follow these procedures in the event that a senior is under the influence, and/or in possession of drugs and/or alcohol:

The parent/guardian of the senior will be contacted.
The senior will be held in an area separate from the party.
Under no circumstances may any senior who has used or is in possession of drugs and/or alcohol be allowed to leave the party except in the custody of the parent, guardian, or the Windsor Police Department.

Release and Assumption of Risk Agreement

The below identified Senior wishes to participate in the Windsor High School Project Graduation Party. I understand that there are risks and dangers incidental to the below identified Senior's participation in this event and that there is a risk of injury or illness to the below identified Senior, including but not limited to Covid-19. I hereby give consent for the below identified Senior to participate in this event and voluntarily assume all risk and dangers, known or unknown, associated with participation in this event.

In return for the Windsor High School Project Graduation permitting the below identified Senior to participate in this event, I hereby release and waive on my own behalf, on behalf of the below identified Senior, and on behalf of any persons claiming by or through me, or through the below identified Senior, any and all claims or causes of action for ordinary negligence which I or the below identified Senior may have against the Windsor High Project Graduation, its officers, directors, agents, employees and volunteers arising out of or including death, which the below identified Senior may suffer while taking part in the event or any activity connected to the event.

I understand that if the below identified Senior is a legal adult, then they may submit this document on their own behalf. I also understand that if the below identified Senior is a minor, their Parent/Guardian must submit this document.

BY COMPLETING THE INFORMATION BELOW, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Release and Assumption of Risk Agreement, I understand it and sign it voluntarily as my own free will; I am at least eighteen (18) years of age and fully competent; and I execute the Agreement for full, adequate and complete consideration fully intending to be bound by same.
