Extra Curricular Information:

Extra Curricular activities commence as of Week 2 of Term 1, 2025 and run all year (unless advised). The activities are delivered within a designated area within the school. Children will participate in the normal OSHC program before and after the extra curricular session. Children will be escorted to and from the activity and supervised by OSHC educators for its duration (with the activity being delivered by external staff from the company).

The Child Care Subsidy will be applied to the additional extra curricular fee ($10).
Important Information:

- The start and end dates specified within this form must be for a minimum duration of 4 weeks and no longer than 1 year.

- All permanent bookings require 2 weeks' notice to cancel a session/s (without charge). Please complete the cancellation form if you wish to cancel a booking.

- All permanent bookings will cease during Vacation Care (inc. Public Holidays) and resume at the start of each term (if specified).

- Once this form is submitted, bookings will not appear instantly within the OWNA app as permanent bookings are entered manually by our admin team. Please allow for at least 2 business days for these bookings to appear in your app.
