I. the undersigned, am responsible for the care and financial responsibility of the pet mentioned above and that I do hereby give Dr. Keith E Gudgel, or his agents, servants, and/or representatives' full and complete authority to perform the surgical procedure described above. Furthermore. if any unforeseen condition arises calling in his/her judgment for procedures to be performed on my pet in addition to or different from those now authorized in order to save my pet's life or prevent a life threatening situation on my pet, I further request and authorize him/her and his/her assistants to perform whatever procedures deemed advisable to help my pet. I acknowledge that no guarantee or assurance has been made to me as to the results that may be obtained. I consent to the administration of anesthesia, pre-anesthetic agents, antibiotic, fluids, pain relieving medications or any other drug deemed necessary in the medical interest of my pets life.

    LABORATORY TESTING: It is in the best interest of a pet undergoing anesthesia and a surgical procedure(s) to perform pre- operative blood testing to screen for certain pre-existing conditions which might endanger the pet's life.

  • IV CATHETER/FLUID THERAPY:

  • Intravenous fluids help regulate blood pressure and administer medication in an emergency situation.

     

    IV FLUIDS AND CATHETER PLACEMENT $75 - $120

  • PAIN MANAGEMENT: Animals receiving pain control are more comfortable after surgery and relieving pain aids in the healing process.

     

    POST - OP PAIN INJECTION: (given immediately after surgery) $42.50

  • POST OPERATIVE PAIN MEDICATIONS

  • PATHOLOGY: This test is performed on a tissue sample and determines the nature (cancerous or benign) of the growth. $235.00

  • ADDITIONALCHARGES: Additional charges may apply due to animal size. behavior. increased surgical/anesthesia time and increased difficulty in the procedure itself. Examples include but are not limited to: Pregnancy/In-heat spay. cryptorchid Initials required castration. large/unruly animals etc.

     

  • CASE OF EMERGENCY: In the event of an emergency one can choose:

    CPR (life saving measures are taken) DNR (do not resuscitate)
