How to Use the Inventory and Requisition form

1. The Alert program is an MD-20 Lions and Lions Clubs International Foundation funded emergency response program.
   As such, only requests for supplies and equipment from a Lions Club(s) will be honored.
2. Most items are consumables and we do not expect their return after use.

Dehumidifiers
Shop Vacs
Emergency lighting
Power strips
HD Extension cords
USB Chargers

3. To request supplies and/or equipment:

Complete the Requisition form with items and quantities requested
Scan and e-mail to: Lion Rick Brisee rbrisee@hotmail.com

• The Alert Committee will do everything possible to have requested
  equipment and supplies available within 24-hours of such requests
• Equipment and supplies will be loaded onto an Alert utility trailer for
  pick-up by the requesting Club. Trailers will be expected to be
  returned in the same condition as delivered. Equipment will be
  expected to be returned in working condition with expected wear
  and tear after use.
• Clubs are responsible for knowing where and to whom equipment
  and supplies were loaned or are located and will be responsible for
  seeing that those items are returned in working order.
• Clubs may be held financially responsible for items that are not
  returned or are returned with discernable damage or in non-working
  condition.

4. The newest utility trailer (14 ft) is equipped with both electric brakes and surge brakes and is designed to be pulled by a mid-size SUV (Jeep Cherokee, etc) or pick-up (F-250, Chevy Silverado, etc) and is rated with a GVW of 4,000#.

LIONS of MD-20 ALERT FOUNDATION

Hold Harmless Agreement: Borrower and recipient Lions Club agrees to and shall
hold the MD-20 Lions Alert Foundation (The Foundation), its officer and Board, and any and all auxiliary and related entities harmless from any liability or claims for personal injury, including death, as well as any and all claims for property damage which may arise from the use of any and all supplies and equipment provided by The Foundation and which may arise from the use by any agent, sub-agent, contractor, independent operator, or recipient of said items.

Borrower and recipient Lions Clubs shall agree to and shall defend its elected and appointed Boards, officers, agents and employees from any actions and suits at law or in equity for any damage caused or alleged to have occurred resulting from use or acceptance of said property or equipment.
