Notes:
1. Application Fee is $150.00 per structure.
2. Permit Fee is charged at $5.00 per linear foot (Height to top of tower).
3. Plans, diagrams and other data shall be submitted in four (4) complete sets with each application.
If the CBA has no objections;
(1) One set will be retained by the CBA;
(2) Two sets will be returned to the owner; one for the owner, the other for the engineer of record.
(3) One set is to be kept at all times at the construction site.
4. Engineers are required to stamp AND sign each page of each set of the documents submitted.
