To apply for water, wastewater and reuse water services please complete the application and submit proof of ownership (either a PDF or picture of your Settlement Statement or Special Warranty Deed) and a copy of your identification. If you are renting your home you must include a copy of your signed lease agreement.

Your application will not be processed without the required documentation.

Once you submit this application with the required documents we will create an account for you. You will receive an email that will include the total amount due. This amount is made up of a refundable deposit and non-refundable set-up fee.

All application charges must be paid within one business day, or service may be disconnected without further notice.

I understand that I am responsible for prompt payment for all utility service charges rendered to the address listed above including any collection fees associated with unpaid balances. I agree to abide by current and future rates, regulations, policies and procedures for potable, waste and reuse water services as established by the governing boards of both SID and the City of Westlake.

I UNDERSTAND THAT MY ACCOUNT IS NOT ACTIVE UNTIL I HAVE PAID ALL APPLICABLE DEPOSITS AND ACCOUNT SET UP FEES. THESE CHARGES ARE DUE WITHIN ONE (1) BUSINESS DAY FROM THE DATE ON THE APPLICATION OTHERWISE THE SERVICES WILL BE DISCONNECTED. I MUST ALSO PROVIDE PROOF OF OWNERSHIP OR A SIGNED LEASE AGREEMENT PRIOR TO ACTIVATION OF SERVICE.

Only authorized personnel are permitted to turn on/off service at the meter. Violations may result in fees, fines or discontinuance of service. Tampering with meters is a criminal offense; violators will be responsible for all damage, including parts and labor.

SID IS NOT RESPONSIBLE FOR LOSS OR DAMAGE RESULTING FROM STARTING SERVICE. ALL INSIDE AND OUTSIDE OUTLETS (FAUCETS) MUST BE OFF WHEN SERVICE IS CONNECTED.
