Request Form

Please use this form to place a request with us. All information collected is required by Big Group Hug for packing & statistical information only. Once your request has been processed our Request Team will contact you.

Note:
1. If your client has more than 1 child, please use a separate page for each child requiring material items. This ensures that the correct number of items are provided for each child and that they are tailored to their age and gender.

2. If your client has 7 or more children, please make a separate request for the remaining children.

3. We collect, hold, use and disclose personal information so that we can perform our functions as a not-for-profit organisation and provide you and your client with the best possible services and outcome. Information may also be used for statistical and promotional purposes to further our work, in which case it will de-identified. Information provided will be kept confidential and will abide by our Privacy Policy available here.

4. Please review the request before submission to ensure the request captures your client's needs accurately.

If you are requesting through a child care, pre-school, school, or education provider:

• If requesting through a Child Care, please select 'Child Care' against 'Organisation Type'.
• If requesting through a Pre-School or Kindergarten, please select 'Pre-School' against 'Organisation Type'.
• If requesting through a Primary or Secondary School, please select 'School' against 'Organisation Type'.
• If requesting through a Post-Secondary Education Provider, please select 'Education' against 'Organisation Type'.
• Once you select your 'Organisation Type', start typing the name of your Organisation to locate it in the dropdown menu.
• If your organisation is not in any of the above drop down lists, please select 'New Organisation Registration'. After you place your first request with us, your organisation will be added to the appropriate 'Organisation Type' drop down list.

 

If you are not requesting on behalf of one of the above organisation types, please check if your Organisation is in the 'Other Independent Organisations' list first.

If it's not listed in the 'Other Independent Organisations' list:

• If requesting through a Maternal Health Service(MCH) or local council, please select 'Local Council Organisations' against 'Organisation Type'.
• If requesting through a hospital, please select 'Hospital' against 'Organisation Type'. 

 

If your organisation is not in any of the above drop down lists, please select 'New Organisation Registration'. After you place your first request with us, your organisation will be added to the appropriate 'Organisation Type' drop down list.

Once notified by our Request Team, you will be able be collect your request from the Big Group Hug Thomastown warehouse, Airport West Hub or Tarneit Hub. Requests cannot be collected by your client.

Alternatively, we offer a request delivery service with a charge of $60 including GST per location / delivery address.
Delivery is only available within 40km of Thomastown and can be to you or directly to your client. The requesting agency is expected to fund this small cost.

Please note: If the request is being delivered directly to your client, you will need to confirm that your client will be present to accept the delivery, and let us know that this has been confirmed.
