Nutcracker Dance Camp Registration Form
I am so excited to be able to share one of my most favorite Holiday traditions with your dancer.

We will explore both acts of this famous ballet, with games, story time and of course, DANCING!

The event is open to dancers and non-dancers. Dancers who are currently in a class, or dancers who used to be in a class.

This Nutcracker Camp is an hour and a half long camp that will be held on July 2,2024 from 5:00-6:30pm.

Cost for this camp is $30/dancer. Invoice for the camp will be sent, via the SDA PayPal account, to the email provided in the registration

DRESS CODE: since this event is open to anyone, dance wear is not required. Dancers will be expected to wear clothing that they can move freely in. Ms. Kim will bring a small collection of dance shoes, but there is no guarantee that your size will be available. If your dancer does not have dance shoes, they can particpate in street shoes with non-marking soles, or in their socks.

The Nutcracker Winter Break Camp will be held at the MAIN BALLROOM of the Celebrity Ballroom Dance Facility. Located at: 2727 Canton Rd. Suite 520, Marietta, GA 30066

We, the undersigned parents/guardians, upon signing this agreement do hereby acknowedge that the activities I have requested my child in my care to participate in may be stressful on the body and carry with them the risk of physical injury. On behalf of my child as his/her parents and/or legal guardians, I assume the risk and agree that the Stafford Dance Academy shall not be liable in any way for injuries sustained or loss of property during attendance at the dance studio, any of it's related functions, as a participant or an observer on or off of the premises. Student must be covered by their own family health insurance.

We understand that Stafford Dance Academy from time to time, produces promotional material about it's program. We understand that as a participant the above mentioned minor may be included in a video or photograph taken at the studio or performance venue. We hereby grant to Stafford Dance Academy, it's successors, assignees, licenses, sponsors the exclusive right to photograph and/or video participants and further utilize participants name, face, likeness, voice and appearance as part of the program, and in advertising and promoting the program, without reservation or limitation. This explicitly allows use in any social media campaign or posting, such as Facebook, Instagram, Twitter, or the like. In granting this license, I understand that Stafford Dance Academy is under no obligation to exercise any of its rights, licenses or privileges herein granted by participant.

An invoice for registration payment for this event will be sent via the Stafford Dance Academy PayPal page after this registration form has been received. Invoice will need to be paid prior to the event start in order for my dancer to be able to participate. No dancer will be turned away, but, a $10 late fee, per day, will be added to the invoice if dancer participates prior to paying invoice.
