As a vendor, I expressly assume the risk and legal liability related to my activity with SoLoved. I waive and release all claims for injuries, possible Covid-19 exposures, damages or loss which I may sustain as a result of my participation. I understand that I am solely responsible for my own safety both on the premises and in traveling to and from the work sites.
I warrant that I have no physical or psychological medical conditions that would create risk(s) for others or myself in connection with this vendor event.
By submitting the following form, I hereby give my permission for SoLoved to use photographs or video footage taken during my time as a vendor in publications, presentations, websites and/or other promotional materials. I gladly give this authorization to support the efforts of SoLoved to continue until terminated in writing by either party, (emails accepted) to soloved316bvp@gmail.com
There are no vendor refunds, unless the event is cancelled by the host.
