By signing this application, you agree to the following:
1. ADOPTION FEE: Payment of an adoption fee. This fee helps to defray the expenses incurred for this pet(s) while in the organization's care such as food, grooming, providing monthly heartworm and flea prevention, and veterinarian expenses (shots, testing (and treating if necessary) for heartworms and 'or parasites, spaying/neutering, etc Any donation you can make in addition to your pet adoption fee would be greatly appreciated! ALL MONEY IS USED TO TREAT AND SAVE ANOTHER LIFE! 2. GRACE PERIOD: If within 2 weeks of your adoption, you wish not to adopt this/these pet(s), the Organization will reclaim the pet and either refund the adoption fee or provide you with another pet (s If after two weeks, and for the duration of the life of the pet(s), you should no longer wish to keep this/these pet(s), it/they MUST be returned to the Organization, with no refund. Also, if this pet is found to be in anyway unsuitable within the 1-year period of this contract, Road Dogg Rescue reserves the right to take possession of the Named pet immediately. 3. CARE OF THE ANIMAL: You agree to treat this/these animal(s) as a household pet, companion and family member. You agree to provide the pet(s) with daily wholesome food and clean water, affection, adequate exercise and a minimum of a yearly veterinary care. (in addition to any emergency care that may be required) The pet(s) MUST ALWAYS wear an ID tag with local city/county. The pet(s) MUST be primarily an inside pet(s It/they MUST NOT be left outside, even in a fenced in yard, for long periods of time unattended. The pet(s) MUST NOT be allowed to roam outside a fenced area. The pet(s) MUST NOT be subjected to any form of cosmetic surgery including, but not limited to, ear cropping, tail docking, declawing or de-barking. The pet(s) MUST NOT be used for any experimental or fighting purposes whatsoever. These are rescues and they deserve peace. 4. TRANSFER; EUTHANASIA: You agree to not abandon, give away, sell or dispose of the pet(s) in any way, and to notify the Organization immediately at any time you determine that you no longer want to, or no longer can, humanely keep/care for the pet(s), so the Organization can take the pet(s) back and arrange other adoption(s You agree to NOT euthanize the pet(s) except in the case of the pet(s) terminal illness/injury, or old age accompanied by pain and suffering, and in that case, the euthanasia must be performed by a licensed Veterinarian in a clinic or hospital. 5. LOST PET(S): You agree to make a serious effort if the pet(s) become lost or missing by immediately filing reports with the local police, animal control authorities, animal shelters, SPCA's and local veterinarians; and contacting the Organization for additional advice. (Please get a collar or tag with your current phone# and address for easy identification. Collars are for identification purposes and should always be on your dog. A harness is much more humane way to keep your dog on a leash. Easy to handle and keep your dog under control without hurting him/her If you do not plan on walking your dog, leave him/her here. 6. VISITATION AND PERIODIC INSPECTIONS: The Adopter agrees to provide the Organization and/or it's representative with information concerning the health and well being of the pet(s) upon request and to allow the Organization to conduct a follow-up on the pet(s) condition and treatment at any time. These follow ups will occur with notice for 1 year after pet(s) have been adopted. 7. ENTIRE AGREEMENT; MODIFICATION; BINDING EFFECT: This Agreement is the entire agreement between you and us and supersedes any prior understandings between you and us with respect to the subject matter of this agreement. No modification of this agreement will be valid unless signed by both you and us. This agreement is binding upon your and our heirs assign, successors, personal representatives, and executors.
