If you would like the Club to initate a free volunteer Working With Children Check application for you, please complete and submit this request form.

Please note:

The SA Department of Human Services (DHS) Screening Unit will send your application to the email address provided on this form. Please ensure it is an inbox you check regularly.
If not completed, your application will expire 4 weeks from date you receive the DHS email.
Applications for members aged under 18yrs must be printed; signed by a parent/guardian; and identification must be verified in person by a verifying officer.
Applications for members that do not hold Australian citizenship require identification to be verified by a Club verifying officer.
