PERMISSION:

My child has permission to attend Faith Rocks Performing Arts Academy ("FRPAA"). I understand that my child will need to be available to attend 30 minute class or session. (If there are any changes to the schedule you will be notified)

Terms/Conditions:

Enrollment payments are final sale. Please note, a 24 hour notice is required to make up a cancelled session. If the student is more than 10 minutes late, without 24 hours notice, that session cannot be made up.

MINOR VIDEO, PHOTO AND WEBSITE RELEASE FORM:

I give Faith Rocks Performing Arts Academy permission to publish in print, electronic, or video format the likeness or image of my child and the art/works created by my child. I release all claims against the "FRPAA" with respect to copyright ownership and publication including any claim for compensation related to use of the materials. *When images are published FRPAA will take cautionary steps to provide minimum identifying information and will not use full names, street or mailing address, emails or phone numbers.

WAIVER OF LIABILTY: This waiver of liability must be received before the student can participate.

I, the undersigned or parent/legal guardian of (child's name above) do herby waive, release, and discharge all claims against the "FRPAA" for damages, death, personal injury and property damage as a result of participation in this program. By signing this release, I understand that if the student is hurt during the program I have waived my right to hold a lawsuit against "FRPAA" or "Coach" Shea Edwards. This release is intended to discharge and hold harmless the "FRPAA" instructor from liability.
