Banquet Request Form

Thank you for choosing Pizza a' fetta for your very special event.

How the process works:

1. First, you will fill out the form and provide us with the information needed to create the quote for the event.

2. We will review the information and send you a response via email with the initial quote including any details you may have requested.

3. after all the quotes had been accepted by both parties, a final quote will be sent for your review and a deposit will be required to book the event.

Correspondence:

We ask that your party choose one person to correspond with us during the creation of the event so that we minimize any confusion. James Faurentino will be the only person that you will correspond with from Pizza a' fetta so please don't call the store and talk to the staff about the event as it will only confuse details.

If there is a phone conversation between both parties during the creation of the event then all details must be provided via email after the conversation. Failing to do this may result in details being missed in translation. PLEASE RESPOND VIA THE CHAIN EMAIL AND DO NOT START A NEW EMAIL PLEASE DURING THE PROCESS.

Deposit:

A $200.00 deposit will be required to book the event and will be applied to the final total do at the event.

Cancelation:

We require two weeks prior to the scheduled event date for the cancellation and for the deposit to be refunded in full. If you cancel short of two weeks the deposit will not be refunded.

Cannon Beach Location:

This location is on the North end of town and can serve up to 35 to 40 guests with a full sit-down menu and is located on the upper floor of the restaurant.

Seaside Oregon Location:

This location is located on the east side of the river off-Broadway at the start of the downtown seaside area. This location Banquest Room is currently under construction and scheduled to be completed in 2022
