• The Permit is valid for one (1) day only.
• This form shall be properly filled out at least one (1) business day before the scheduled delivery/pull-out date and must be submitted before 5:00pm (cut-off).
• Please observe proper attire. Only personnel wearing shirts, pants, and closed shoes shall be allowed to access the building.
