I am SO excited that you are taking the first steps towards your dream hair! Please fill out this form in as much detail as possible. This consultation form is designed as a preliminary screening process to best prepare us for our first meeting. This allows me to get a good picture of your hair history and the overall experience that you are looking to have with your color! There are two pages to this form - the first part is all about your hair and hair history, and the second portion is your contact information. After completing this form, you will receive an email from me with current availability an options for your in-person consultation visit. Please note that service pricing and color matching can only be determined at the time of your consultation and when I see your hair in person! I understand that at the time of booking my consultation, Lacy will require a credit card to book my appointment. My credit card will be charged $25 in the event I dont show up or call to reschedule my consultation time or $50 per service for all future appointments.
