Please submit this form after successful graduation to a new skating level OR if you would like to make a change to your current class schedule (i.e. move days). Form must be submitted no later than the 7th of the month to make the change for the current month. If submitting after the 7th, the change will take place the following month.

I understand that if I am graduating to a class that is more expensive my monthly billing amount will increase. I understand that if I am graduating out of the Intro to Skating class into a Snowplow Sam class or above, these classes only meet 1x per week for the monthly cost listed in the rate chart above.

I have reviewed the Class Fees chart and understand the new amount I may be charged next month. I agree to the possible increase in my montly billing amount based upon the class that I graudated to and the number of times per week that I note on this form my child will be attending.
