Consent Form for Online Survey
You are invited to participate in the following online survey as part of a research project to understand:
What arts provisions are provided by schools as part of their curriculum;
If parents are aware of the benefits of The Arts for Health and Wellbeing;
If parents would be interested in schools providing Creative Arts and Health workshops to support the emotional and social development of children.
This survey is being conducted by Donna Sheppard, a student at The University of Derby. Questions are multiple choice and should take approximately 5 minutes to complete.
Participation
Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time. You are free to decline to answer any question you do not wish to answer for any reason. Once your survey is submitted you will no longer be able to opt out, and your responses will be included in data analysis.
Benefits
Your responses may help us to learn more about the curriculum schools offer; if this includes The Arts, and if Arts for Health Workshops should be included. Your responses will feature in data that may be published.
Risks
There are no risks in participating in this study.
Confidentiality
There are no personal identifying questions in the survey. Your survey answers will be sent to a link at Jotform.com where data will be stored in a password protected electronic format. Your responses will remain anonymous; no one will be able to identify you or your answers.
