This is a contract between Pet Grooming by Purple Pooch Palace of Perfection (P4) and the pet owner whose signature appears below   (hereinafter called   “OWNER”).    “Groom”, “groomed”, or “grooming” includes any services and/or procedures performed by Purple Pooch Palace of Perfection (P4).

 

1.    OWNER  agrees to pay for all grooming services requested,  and all veterinary costs for the pet while in the care of  Pet Grooming by P4.

 

2.    OWNER agrees that the pet shall not leave Pet Grooming by P4 until all charges are paid to Pet Grooming by P4 by OWNER.  If the pet is not called for by closing of business day of the stated pickup date, the pet will be placed with a Humane Society or Rescue group, we are not a boarding facility.  The Owner shall be liable for any unpaid charges to P4 and other services.

 

3.    If the pet becomes I; or if the state of the pet’s health requires professional attention while the pet is being groomed, Pet Grooming by P4 in its sole discretion, may engage the services of a veterinarian, or your veterinarian by request by OWNER, you will be contacted immediately, administer medicine, or a special diet, or give other requisite attention to the pet and the expenses thereof shall be paid by the OWNER if use P4 Veterinarian.

 

4.    All pets are groomed at/by Pet Grooming by P4 without liability on the part of Pet Grooming by P4 for loss or damage from injuries while pets are being groomed, disease, death, running away, thief, fire, injury to persons, injury to other animals, damage to property by said pet, fence climbing, or other unavoidable causes, due diligence and normal care and caution having been exercised.

 

5.    It is expressly agreed by OWNER and Pet Grooming by P4 that the liability of Pet Grooming by P4 shall in no event exceed the lesser of the current chattel value of a pet of the same type/breed or the sum of $200.00 per animal groomed.  The OWNER further agrees to be solely responsible for any acts or behavior of said pet while it is in the care of Pet Grooming by P4.

 

6.    OWNER ensures that all State of Michigan vaccinations for the stated animal listed in this contract are current.   Vaccination or Immunity for parvo/distemper, rabies, corona, and proof of immunity for Bordetella.

 

7.    By signing this Contract and leaving your pet with Pet Grooming by P4, OWNER certifies to the accuracy of all information provided about the pet, that the pet is current on all vaccines required by the State of Michigan, and Pet Grooming by P4, that pet has not been exposed to rabies or distemper within 30 days before any grooming appointment, and that OWNER is the legal owner of a said pet, free and clear of all liens and encumbrances and assumes financial liability for all payments for this agreement.

 

8.    This Contract contains the entire agreement between the parties.  All terms and conditions of this Contract shall be binding on the heirs, administrators, personal representatives, and assigns of the OWNER and Pet Grooming by Purple Pooch palace of perfection (P4).

 

9.    Any controversy or claim arising out of or relating to this Contract, or the breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this contract, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgement upon the award rendered by the arbitrator may be entered in any Court having jurisdiction thereof.  The arbitrator shall, as part of his award, determine an award to the prevailing party of the costs of such arbitration and reasonable attorney’s fees of the prevailing party.

 

This Contract applies to the grooming appointment at Pet Grooming by Purple Pooch Palace of Perfection (P4), and all subsequent grooming appointments from the date it is signed by the OWNER.

Matted Hair Agreement

Your pet’s hair is matted.  Your pet will most likely be shaved down as close as necessary to remove the matted hair.  ALL matted hair will be removed regardless of where the matted hair is located including the head, face, and anywhere else is needed.

Also, because of the difficult processes involved in removing matted hair from your pet’s skin may become irritated.  Signs of irritation can include redness and/or swelling, particularly on the pads or bottom of the feet.  Any area of the skin can become irritated when matted hair is removed from the skin.  Also, small hematomas can develop on the ears as well as other areas.  This irritation is caused by the closeness of the clipper blade to the skin.  You might want to consider having a veterinarian examine your pet before having the matted hair removed. 

We will do all that we can to make your pet comfortable.  However, irritated skin is common when matted hair is removed.

By signing below you agree to this policy and you understand that, because of your pet’s matted hair, irritation may occur and we may not be able to achieve the specific style of the groom that you want.
