Vaccination Required Party Submission

Please Note: This event is only open to individuals who have completed the COVID-19 Vaccine Process and proof of vaccination will be required.

Thank you for your interest in our event. These parties are invite only, and approval is required. If you already have contact with a host, please text them, if not please submit this form. Once submitted you will be contacted by a host via text messege the day of the party. No submissions will be considered if recieved after 7PM on the day of any event.

HOUSE RULES
1. Please treat everyone attending with respect, discretion, and honesty! Please note: A raised hand means "Not Interested"

2. NO means NO. Simple, and only required to be said once. Same goes both ways, if someone tells you no, please respect their wishes.

To get your submission rolling, please make sure all information above is accurate. Please attatch minimum of 3 pictures below. Face picture is required. Proof of Vaccination is also required, you are welcome to edit out any information from Vaccination Record you would like. If you are not submitting a photo of your vaccine record, please be prepared to show this at the door. We do not share anyones pictures.
