I, named above, as Parent/Legal Guardian of the minor named above (the "Minor") hereby grant the Community Education Building Corp., its agents and assigns ("CEB") the absolute and irrevocable right and unrestricted permission to use, reproduce, publicly display and publish any photographic or videographic images taken of me and/or the Minor, and all derivative works thereof, including any likeness or image, in conjunction with my or the Minor’s name, in print, electronic and all other media, for illustration, promotion, art, publicity, advertising or any other purpose for an indefinite period of time.

I give unrestricted permission for images, videos, and recordings of the me or the Minor to be used in print, video, digital and internet media. I agree that these images and/or voice recordings may be used for a variety of purposed and that these images may be used without further notifying me.

I further acknowledge that neither I nor the minor will be compensated for these uses and the CEB owns all rights to the images, videos, and recordings, and to any derivative works created from them.

I hereby waive any right that the minor or I may have to inspect or approve any materials that may be used in connection therewith, or the use to which it may be put.

I hereby release, discharge and agree to hold harmless the CEB from all liability in connection with any such activities and materials, as well as any publication thereof, including with respect to any claims relating to copyright ownership, publication, defamation, infringement of moral rights, invasion of privacy and publicity, and any claim for compensation related to any ise of such materials.

I herby warrant that I am if full age and have every right to contract for the minor in the aboveregard. I state further that I have read the above authorization, release and agreement prior to its execution and that I am fully familiar with the contents hereof.

The Release expresses the complete understanding of these parties.
