WAIVERS & General Policy
Gallatin K9, LLC will endeavor to create as safe environment as possible for the training and boarding care of the dog(s) identified as (the dog), and will offer a safe, sound and responsible training, care and post-boarding training instructions. Owner agrees, however, that all animals may exhibit unpredictable behaviors and that Gallatin K9, LLC will not be liable for the actions of the dog that are outside the control of Gallatin K9, LLC training Owner agrees to allow random stool samples to insure safety of the kennel. Owner agrees that the recommendation by Gallatin K9, LLC training of any product or service is NOT a guarantee of performance or Owner's satisfaction with that product or service. Gallatin K9, LLC agrees to indemnify and hold Owner harmless from and against any claims of injury, expense, costs, loss of damages caused by the gross negligence or willful misconduct of Gallatin K9, LLC training while the dog is in sole custody, care and control of Gallatin K9, LLC training. Owner acknowledges that Gallatin K9, LLC has no control over the dog when the dog is not in the exclusive custody, care and control of Gallatin K9, LLC training. Owner agrees to indemnify and hold Gallatin K9, LLC training harmless from and against all claims of injury, expense, costs, loss of damages caused by the actions of the Dog when the Dog is NOT in the sole custody, care and control of Gallatin K9, LLC training. Owner fully understands the tools and techniques that are used to train and modify the behavior of their dog and agrees to the tools and techniques which include Prong Collar, Bark Collar, Remote Training Collar, Slip Lead, Pet Convincer, Muzzles, Flat Buckle Collar, Martingale Collar. All dogs must have proof of vaccinations and present either before or day of drop off. If shots have not been updated, cannot allow dogs to be housed at the facility. Required shots rabies, Bordetella and fecal float. ****Payment and cancellation Policy**** Payment Policy: Payment 100% in full at time of signing contract to book board and train program. Cancellation Policy: If the client chooses to NOT continue training after receiving approximately 50% of services, a refund WILL NOT be issued. If the trainer feels that the client will not listen to training advice or follow instructions, Gallatin K9, LLC can at sole discretion, choose to cancel the training contract and issue a partial refund will be given minus $500.00. Photos/Video Release (All Clients) I hereby assign and grant to Gallatin K9, LLC the right and permission to use, reproduce and distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representations(s) and/or sound recording(s) made of me and /or my dog(s) at any time during the training and/or boarding of my dog(s) by Gallatin K9, LLC, and I hereby release Gallatin K9, LLC, or any person that represents Gallatin K9, LLC from any and all liability resulting from such use, reproduction, distribution, and publication. I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and or sound recording(s) without limitations at the discretion of Gallatin K9, LLC and I specifically waive any right to any compensation I may have for any of the foregoing. Physical Activity Agreement: Your dog will be staying in my home and in its crate, and not living outside. Your dog spends a good amount of time in the outside world doing lots of things around lots of natural objects, dirt, grass, cement, trees, fencing, bushes, swimming, interacting with lots of other dogs, and humans. It is likely that your dog is going to get scratched, bumped, or bruised. Your dog will most likely get some wear and tear on him/her. Things are bound to happen even though we have taken special care in designing our facility and maintaining a high standard of safety and cleanliness. This is just like when you were a kid going out to play and would come back home with scratches or cuts, because you were having fun and exposed to the outdoors. As Owner, you acknowledge and understand that there are certain risks involved in pet training, boarding, and care. You release, indemnify, and agree to hold DePaul K9 Academy harmless from any and all manner of damages, claims, loss, liabilities, costs or expenses, causes of actions or suits, whatsoever in law or equity, (including, without limitation, attorney’s fees and related costs) arising out of or related to the services provided by Gallatin K9, LLC. Gallatin K9, LLC will endeavor to create as safe an environment as possible for the training and boarding care of the dog(s) identified above (the "Dog" and will offer only sound, safe and responsible training, care and post boarding training instructions. Pick Up/Drop Off Board and Train-dogs will be picked up and dropped off by owner only, unless written permission is given to Gallatin K9, LLC for another person to handle your dog. Pick Up- We will work with you and your dog to show you the type of work and training that took place while your dog was with us for B&T. We will also explain how you are to continue implementing the training into you and your dog's every day life. Follow up Lessons- This will help transition of your dog back into your home and help maintain a high standard for behavior we have at our facility. 2 Sessions will be offered for each B&T. Drop Off-We will schedule a time for the drop off for B&T. Your dog will need to be on a leash/collar that the dog cannot slip out of when getting out of your car and onto our property, then to a staff member. It is possible your dog will be scared, nervous or excited and this can lead to your dog trying to escape the collar and possibly running into the road which is unsafe, so be sure you have hold of your leash and the collar cannot slip off your dog. The Hand-Off When you get to our place and hand off the dog to us, you are NOT to hug your dog, kiss or bend down and make high pitch sounds to your dog, this just causes your dog to get too excited about your leaving.
