I hereby entrust The Playground staff to care for my pet, Pet's Name*, while playing at doggie daycare, and agree to all of the terms outlined in the Doggie Daycare Rules and Overview document within my provided information folder. If I am unavailable to pick up my pet(s) past the date reserved, I understand my pet(s) may be moved to an alternate available space. I authorize Lafayette Veterinary Care Center to do what they deem necessary for the health and well-being of my pet(s) and agree to pay for any and all expenses relating to said services. I am aware that my pet, despite being vaccinated, could still be susceptible to contracting airborne illnesses, such as kennel cough. My pet will be examined for fleas at check-in and treated if necessary at a charge of $10. I agree to pay the daily daycare rate of $25 /day (unless a punch package is purchased) for each pet(s) playing in the play yards and resting in a loft/condo. I also agree to pay any late fees required for pick up after The Playground has closed for that day.
I agree that I am solely responsible for any damages that the above-mentioned pet(s) may cause through malicious behavior or improper conduct. I also agree to give permission for my pet(s) to have unrestricted access to our equipment at our facility during their stay at Lafayette Veterinary Care Center. Therefore, agreeing to release Lafayette Veterinary Care Center from any liability for damages or harm that my pet may cause to my personal belongings or to themselves during their stay.
Lastly, I am aware that I will be required to make my pet's daycare reservations ahead of time. LVCC reserves the right to refuse services due to limited capacity and the safety of the daycare pets.
