Liability Waiver I am voluntarily participating in the tournament that can be strenuous and subject to risk of serious injury. MVA ensures that you consult with your guardian before participating in these activities. You agree that by participating in these physical activities, is at your own risk. I HEREBY, for myself, my heirs, or personal representatives, (shall also include player's parents or guardian if Player is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in the Activity. I HEREBY release and forever discharge MVA, members, officers, volunteers, from any physical or psychological injury that I may suffer as a direct result of my participation in the aforementioned Activity. I FURTHER AGREE to indemnify, defend and hold harmless the organization against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney fees and any related costs. I FURTHER ACKNOWLEDGE that organization are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of organization. Both participant and MVA agree that this agreement is clear and unambiguous as to its terms.

Photo/Video Release I hereby grant the MVA permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web- based publications, without payment or other consideration. I understand and agree that all photos will become the property of the MVA and will not be returned. I hereby irrevocably authorize the MVA to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the MVA from all claims, demands, and causes of action which I, my heirs, representatives or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE.
