  • Dear Parent / Guardian,

    Your child is being offered the opportunity to take part in a range of outdoor activities throughout the year.

    These sessions will be based locally and nationally within remote and ‘wild’ areas.

    There will be a maximum of 4 young people with 2 members of staff/adults. To enable the children to participate with these exciting outdoor experiences we would require parents/carers to sign this form.

    Please tick each option to indicate if you wish your child to participate in the activity. In the case you do not wish your child to participate, please leave blank.

  • Please ensure that your child:

    • Is dressed in suitable / old trousers and jackets. Clothing is likely to get wet and muddy on session.
    • Is wearing appropriate warm clothing including hats and gloves.
    • Is wearing wellington boots, walking boots / shoes.
    • Has been to the toilet before session.

    Whilst out of school your child will be under the supervision of TAO Activities. All council school polices and procedures will be adhered to.

    We hope that you will agree to your child taking part in these sessions and ask that you complete the attached parental consent form.

    Parental Consent Form - Drop Off / Collection Procedures
  • All young people must be collected from and dropped off at their home address in the presence of an adult. 

    Requests to change the collection / drop-off location of a young person must be made in advance and approved by the school/local authorities beforehand. ADHOC requests will generally be denied on safeguarding grounds unless otherwise approved by TAO Activities management team. 

    It should be the standard practice to inform a staff member either prior to or during the session if the circumstances relating to pick-up and drop-offs have changed or are likely to change. 

    In the event an adult is not present during the pick-up or drop-off time of a young person, prior parental permission will be required to release the young person from the care of TAO Activities. This you must agree to below.

  • I give permission for my child to be dropped off at their home at the end of the session if there is no adult present to hand over to.
  • I understand that in circumstances whereby I have not provided permission for my child to be dropped off at home alone, should a young person need to be returned, e.g. due to poor/unacceptable behaviour, then I understand I must return to the property for an emergency drop-off immediately.
  • I take full responsibility and any liability for granting permission for my child to be collected and dropped off at the home address and have provided the means for them to access the inside of the property independently, e.g. a key.
  • I understand that I can change the permissions outlaid above at any time either in writing to a TAO Activities staff member or through the completion of another parental consent form.

    Parental Consent Form - Prescribed Medications
  • I give authorisation for TAO staff to assist with the administration of medication as prescribed by a GP or other authorised prescriber. The prescribed medicines will be detailed within an accompanying MARS form.

  • Occasionally, we may take photographs of the young people in our provision.  We use these images as part of our provision displays and sometimes in other printed publications. We will also use them on our provision website, Facebook page, Instagram and Twitter account.
     
    If we use photographs of individual young people, we will not use the name of that child in the accompanying text or photo caption.  If we name a young person in the text, we will not use a photograph of that child to accompany the article. If a child has won an award and the parent/carer would like the name of their child to accompany their picture we will obtain permission from the parent/carer before using the image.
     
    From time to time, our provision may be visited by the media who will take photographs or film footage of a high profile event.  Children may appear in these images, which will sometimes be published in local or national newspapers, or on approved websites.
     
    To comply with the Data Protection Act 2018 and UKGDPR, we need your permission before we can photograph or make any recordings of your child.  Please answer the questions below, then sign and date the form where shown and return the completed form to the school.

  • Conditions of Use

    This form is valid indefinitely from the date you sign it. 
     
    We will not use the personal details or full names (which means first name and surname) of any child or adult in a photographic image or video, on our website, on our social media platforms in our provision prospectus or in any of our other printed publications. 

    If we use photographs of individual pupils, we will not use the name of that child in the accompanying text or photo caption.
     
    If we name a pupil in the text, we will not use an individual photograph of that child to accompany the article.
     
    We may use group photographs or footage with very general labels, such as ‘a science lesson’ or ‘making Christmas decorations’.
     
    We will only use images of pupils who are suitably dressed, to reduce the risk of such images being used inappropriately.
     
    Websites and social media platforms can be viewed throughout the world and not just in the United Kingdom where UK law applies.
