This is my official request to be added to The Breeders Elite Incentive Waitlist.

I understand that if I was already on the Waitlist, I will also complete this form and I will keep my place in line on the waitlist as per previous communications with The Breeders Elite.

For breeders who were NOT nominated in 2021 there will be an initial enrollment fee of $2500+GST for the first year of nomination when accepted into the program off of the waitlist. After the first year the annual enrolment fee will then be $1500+GST for each consecutive year enrolled.
