- Adopter agrees to pay UTH an adoption fee of ${adoptionFee} ($50 off if Adopter chooses to decline trial period)

Adopter understands and agrees that UTH is only transferring such interest and title in the animal as it may have.

Adopter agrees the animal is to be a family companion and agrees to provide the animal with a safe home, adequate food, water, proper care, exercise, love, and attention. Adopter agrees not to violate any laws or ordinances with the animal. {petName} is not to be used as an attack or protection trained animal and will not be used for commercial guard work.

Adopter agrees to keep a buckle collar on the animal with ID tag and current rabies tag.

Adopter agrees to refrain from using pain to train animal, including but not limited to choke, prong, pinch and shock collars, hitting, kicking or choking the animal.

Adopter acknowledges that training and behavior advice are available through UTH.

Adopter agrees to keep the animal safe from danger, including but not limited to aggressive animals and traffic. If at any time the animal is lost, stolen, hurt or killed, the Adopter is to notify UTH by email or phone immediately.

Adopter agrees that the animal will be effectively but humanely confined inside the home or in the yard when no one is at home. When the Adopter is to go away for a prolonged period of time or vacation, Adopter is to prearrange proper care for the animal.

{petName} is not to ride loose in the bed of pickup trucks, convertibles or to be left in a car when air temperatures exceed 70 degrees.

Adopter agrees to have a clean, secure fenced area for the animal. If a fenced enclosure is not available, Adopter agrees to leash walk the animal.

Adopter agrees not to release the animal to or make the animal available to any experimental laboratory or similar organization.

Adopter agrees to have the animal examined by a licensed veterinarian at least once each year and provide all vaccinations and treatments necessary to ensure good health. If euthanasia becomes necessary, Adopter agrees to have it performed by a licensed veterinarian.
- Adopter agrees to release UTH and its members or representatives of any and all possible claims arising from injury or damage caused by the animal to any person or property. Adopter accepts this animal as is with all defects, either observable or unobservable, and assumes all risk for the animal upon signing of this contract.
- Adopter will allow UTH to conduct home visits and phone check-ins at their discretion after adoption in order to check on the animal’s welfare. UTH agrees to provide 24 hours notice for such a visit.
- Adopter understands that animals, while up-to-date on vaccinations, are still at risk of contracting many viruses, parasites, and other illnesses. UTH has informed adopter of any current and past medical issues (see attached), but cannot ensure that future medical issues will not arise and need to be treated. Adopter understands this risk and agrees to treat animal/puppy for any future medical needs through his/her licensed veterinarian.
