  • Staff, Volunteer, and Intern Confidentiality Agreement

  • As a staff member/volunteer/intern of Exodus Refugee-Immigration (Exodus), I understand that I must abide by the following policies which assist in maintaining the client's right to privacy and confidentiality in all matters related to his/her personal life, medical care and client services.

    I WILL NOT:

    • Obtain information which I do not need in order to carry out the duties and responsibilities of my position.

    • Communicate information to any person who does not need it for the same purpose. (See preceding statement)

    • Assist others in obtaining confidential information.

    • Discuss the client's affairs in public areas such as the waiting room, lunch room, hallways or off the Center's premises.

    • Seek out and/or disclose irrelevant or embarrassing facts about any client.

    I UNDERSTAND:

    • That records of all clients must be maintained in a manner that will maintain strict confidentiality, and no documents pertaining to any particular client's case will be discarded without being properly shredded or otherwise destroyed.

    • That no client or client-related information, including without limitation, documents, pictures, notes, files, records, oral information, computer files or similar materials (except in the ordinary course of performing duties on behalf of client) may be removed from Exodus' premises without permission from Exodus' Executive Director.

    • That these policies, which include, but are not limited to the specific examples above, apply to all participants and all employees.

    • That the commitment to confidentiality is to be honored both during and after each employee's tenure with Exodus.

    • That breaches of confidentiality (knowingly or unknowingly revealing information of a confidential nature) are matters of serious consequence that may harm client care, slander a client and his/her family, or lead client, or others, to not choose Exodus for the services they need.

    • That violation of any of these policies may result in my termination from volunteering, interning, or employment, and disciplinary action may be taken without previous warning.

    • That any action which violates the spirit of the participant confidentiality policy will be dealt with in the same manner.
  • Staff members/volunteers/interns of Exodus who are unsure about the confidentiality nature of specific information must ask their supervisor for clarification. Also, as a staff member/volunteer/intern of Exodus, I understand I must abide by these confidentiality policies and that I may be subject to state and federal criminal penalties related to slander or breach of confidentiality. By signing below, I signify that I have read and understand the Confidentiality Agreement stated above.

  • Volunteer Release of Waiver and Liability

    Important: All volunteers of Exodus Refugee Immigration, Inc. must have a signed Release and Waiver of Liability on file.
  • This Release and Waiver of Liability (the "Release") executed today by I, the Volunteer, and I release Exodus Refugee Immigration Inc. ("Exodus"),a nonprofit corporation organized and existing under the laws of the State of Indiana, USA, and each of its directors, officers, employees, volunteers and agents (collectively, "Releasees").

    I, the Volunteer, desire to volunteer with Exodus to provide supportive services to clients of Exodus and engage in activities that may include, but are not limited to, teaching English, mentoring, providing transportation assistance, accompanying to appointments, and assisting with general office work.

    I, the Volunteer, understand that the scope of my relationship with Exodus is limited to a volunteer position and that no compensation is expected in return for the service I provide; that Exodus will not provide any benefits traditionally associated with employment; that I am responsible for my own insurance coverage in the event of personal injury or illness as a result of volunteer service to Exodus; and that I am not acting as an employee of Exodus.

    Ihereby freely and voluntarily, without duress, execute the Release under the following terms:

    1. Waiver and Release. I, the Volunteer, release and forever discharge and hold harmless Releasees from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from services I provide to Exodus.I understand and acknowledge that this Release discharges Releasees from any liability or claim that I may have against Releasees with respect to bodily injury, personal injury, illness, death or property damage that may result from the services I provide to Exodus or occurring while I am providing volunteer services.

    2. Insurance. Exodus does not carry or maintain, and expressly disclaims responsibility for providing any health, medical, disability, or automobile insurance coverage for the Volunteer.

    EACH VOLUNTEER IS ENCOURAGED TO CARRY PERSONAL LIABILITY OR HEALTH INSURANCE PRIOR TO REGISTERING AS AN EXODUS VOLUNTEER. EACH VOLUNTEER THAT HAS AGREED TO TRANSPORT EXODUS CLIENTS IS REQUIRED TO MAINTAIN AND PRODUCE A COPY OF CURRENT AUTOMOBILE INSURANCE.

    3. Medical Treatment. I hereby Release and forever discharge Releasees from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time with Exodus.

    4. Assumption of Risk. I recognize and understand that my time with Exodus may, in some situations, involve inherently dangerous activities. I hereby expressly and specifically assume the risk of injury or harm in these activities and release Releasees from all liability for injury, illness, death or property damage resulting from the services I provide to Exodus.

    5. Photographic Release. I grant and convey to Exodus all right, title, and interest in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by Exodus in connection with my providing volunteer services to Exodus.

    6. Other. As a volunteer, I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Indiana and that this Release shall be governed by and interpreted in accordance with the laws of the State of Indiana. I agree that in the event that any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.

    By signing below, I express my understanding and intent to enter into this Release and Waiver of Liability willingly and voluntarily.

  • Code of Conduct

  • Abuse of power in all its forms is considered gross misconduct and therefore grounds for termination. Exodus clients must at all times be treated with respect and dignity by the persons whose care they have been entrusted. Exodus agents including staff members, volunteers, contractors and directors (for the purposes of this code the term "agent" will refer to any of these parties) must strive at all times to provide a professional and responsive environment of care in which clients are protected from any and all forms of exploitation or abuse. For the purpose of this Code, the term "client" shall be understood to cover all immigrant clients regardless of their status (i.e. refugees, asylees, parolees, victims of trafficking, undocumented, etc).

    1. Sexual exploitation and abuse of a client by an agent of Exodus constitute acts of gross misconduct and are therefore grounds for termination of employment.

    2. Sexual activity with minor clients (persons under the age of 18) is prohibited, regardless of the age of consent recognized by the state in which it occurs. Mistaken belief in the age of a child is not a defense.

    3. Exchange of money, employment, goods, or services for sex, including sexual favors or other forms of humiliating, degrading or exploitative behavior is prohibited. This includes provision of assistance that is due to clients under the terms of the various grants and contracts held by Exodus, including DOS/PRM's Reception and Placement Program.

    4. Sexual relationships between Exodus agents and clients are prohibited since the relationships are based on inherently unequal power dynamics. Such relationships undermine the credibility and integrity of our work.

    5. Where an Exodus agent develops concerns or suspicions regarding sexual abuse or exploitation by a fellow worker, he/she must report such concerns via established agency reporting mechanisms.

    6. Exodus agents are obliged to create and maintain an environment which prevents sexual exploitation and abuse and promotes the implementation of this code of conduct. Managers at all levels have particular responsibilities to support and develop systems, which maintain this environment.

    7. In the event that any agent is made aware of misconduct allegations regarding an agent of the agency a report must be made immediately to the Executive Director of the agency. If the allegations are regarding the Executive Director, then a report must be made immediately to the President of the Board of Directors.
