    Expunction Request Intake Form
  • The Youth, Rights and Justice (“YRJ”) Record Relief Clinic (RRC) provides representation to individuals that request the services of the Record Relief Juvenile Expunction Clinic.

    Upon completion of the Expunction Request Intake Form:

    YRJ will advise you about eligibility for expunction and, if eligible, prepare your application.

    Youth, Rights and Justice ("YRJ") Record Relief Clinic (RRC) has agreed to advise, represent, and act for you by investigating your eligibility for juvenile expunction and, if eligible, applying for expunction. This representation may include consulting, reviewing and verifying orders and/or judgments, and giving legal advice regarding this matter, but does not include representation in court proceedings.

    You have no other attorney helping you with expunction of these records.

    By accepting this representation, you agree that no other attorney is representing you in this matter. YRJ cannot and does not promise a successful outcome. Representation is limited to the investigation and application portion only.

    YRJ may not be able to represent you in court.

    By accepting this representation, you understand that should a court proceeding be required, YRJ may not be able to continue its representation of you in this matter. If a court proceeding is required, YRJ may choose to represent you in the court proceeding but is not required to do so.

    There is no cost to you.

    YRJ has agreed to undertake this representation on a pro bono basis, which means that YRJ will not charge you for any work done by the assigned attorney, legal assistant, case manager, or investigator.

    You agree to cooperate and assist YRJ.

    In accepting this representation, you agree to fully cooperate with YRJ, and to assist YRJ with this matter by:

    A) Providing complete information, including information that will assist in the investigation in this matter;
    B) Maintaining regular contact as is necessary for the conduct of your case, including changes to your address, phone number, or email address;
    C) Attending and being on time for all phone calls;
    D) Promptly notifying the assigned attorney when other people (including but not limited to: law enforcement, the court, investigators, the Department of Human Services, and the Juvenile Department) contact you about the case; and
    E) Responding to reasonable questions from your assigned attorney.

    YRJ’s duties:

    During the scope of this representation, YRJ agrees to

    A) Keep you informed about the status of your case;
    B) Keep all sensitive information provided by you confidential unless authorized by you to disclose it (except that information may be shared with other attorneys and staff of YRJ who are working on the case or as necessary to assist in the case);
    C) consult with you before making any significant decisions about the case; and
    D) not resolve the case without your consent.


    You may end the representation.

    In accepting this representation, you understand that you may end this representation at any time for any reason. If you decide to end this representation, you agree to notify YRJ in writing or orally that you wish to end the representation.

    YRJ may withdraw its representation in some limited situations.

    In accepting this representation, you understand that YRJ reserves the right to withdraw from representing you in certain limited circumstances. These circumstances include, but are not limited to, the following

    (A) where insufficient legal grounds exist to continue a court or administrative action or appeal;
    (B) where you fail to cooperate with the reasonable requests of your attorney;
    (C) where a conflict of interest is discovered or arises which makes it inappropriate for YRJ to continue representation; and
    (D) where you fail to meet the agreements (outlined above) of this representation.

    Representation will continue until:

    YRJ’s representation shall continue until the Court appoints a lawyer, the pending case is resolved, or it is terminated either in writing or orally.
