By submitting this application you have agreed to the Terms & Conditions listed below.
1. Checks are paid out biweekly on Saturday after summit invoices. Pay periods start on Mondays and End on Sundays.
2. Payments will only be made by check or Direct Deposit.
3. Before getting paid all workers must complete this form as well as produce the required documentation.
4. All workers must wear all Required Safety Equipment on the work site.
5. Workers must complete and follow all instructions of Superintendent on the project.
6. All the sub contractors should keep the job site clean and organized, if not, the company will pay a laborer to clean and the sub contractor will be charge $40 a hour.
7. The company will retain 10% of invoice until the project finish successfully.
8. Non Compete Agreement: By signing this form, you are stating that you will not have any relationship or communication with Mastermind Client regarding outside employment, contract work or additional payment. If this agreement is broken, we can withold payment.
