In consideration of being allowed to participate in any way in

Ace 2 Deuce CTA/NJTL and FOBPF, related events and activities, the undesigned acknowledges, appreciates and agrees that:                                                                                  

1)  The risk of injury and exposure to infectious syndromes or disease from the activities involved in this program are possible, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury and/or exposure to infectious syndromes or disease does exist. 

2) Because of the dangers of participation in tennis and fitness training, I recognize the importance of the coach's instructions regarding playing techniques, training, rules of the sport, or to the team rules, and to obey those instructions.

3) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown. EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or assume full responsibility for my participation; and,

4) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any usual significant hazard during my presence or participation. I will remove myself from participation being such to the attention of the nearest official immediately; and,

5) In consideration of Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC permitting me to practice, play or tryout for the program, and to engage in all activities related to the team, including practicing, playing and travel, I hereby voluntarily assume all risks associate with participation and agree to exonerate and save harmless Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC their agents, employees, the Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC athletic staff, the physicians, athletic trainers, and other practitioners of the program teaching me, from any and all liability, claims, causes of action or demands of any kind and nature whatsoever which arise by or in connection with my participation in any activities related to Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC. We are following City, State, and CDC safety guidelines on Covid-19 safe health practices. 

6) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, the Ace 2 Deuce CTA/NJTL, FOBPF,  and DVTC program their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers and if applicable, owners and leasers of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABLILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEE OR OTHERWISE, to the fullest extent permitted by law.

7) I hereby consent to first aid and/or emergency medical treatment of my child by a trained professional. All costs associated with ambulance transport or emergency room/doctor fees will be assumed by me, the guardian.

I grant permission to Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC to photograph, tape, film, or make an audio recording of my child's participation in the Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC Program.

I grant Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC an irrevocable license to produce, copy, display, perform, or otherwise use these materials. I understand that these materials will be used for the purpose of education, promotion, support, and information and may appear in newsletters, newspapers, brochures, magazines, exhibits, fundraisers, Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC publications, social media platforms, Facebook and/or on the Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC websites.

I agree that my child may be identified as a participant in the event and/or program being recorded. I release the Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC program, its employees, directors, and volunteers from any liability connected with the publication, reproduction, release or other use of these materials, and agree not to bring any claims against them growing out of such publication release, reproduction, or other use of these materials.

I allow my child to walk / drive / bike / take public transportation home without the supervision of an adult. I also allow my child to sign out by themselves after tennis and academic programming. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation in Ace 2 Deuce CTA/NJTL, FOBPF, and DVTC programming. I HEREBY RELEASE AND HOLD HARMLESS (Dorothy Vest Tennis Center + City of Jackson) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

This membership entitles your child have a membersip for 1 year. The membership ammedies includes:

• Free court use for the year unless court are in use due to reservations, tournaments, mixers, adult court activties or private facility rental.
• Free water supply for the year.
• Discounts on pro-shop items (racquets, balls, dampners, etc),camps, clinics (inhouse or invited), private coaching, JTT team/group lessons.
• Team shirt free for any season played.
• Discounts on in house, USTA, or PTR training
• Participate in any facility NJTL event at no to low cost depending on type of event.
• Free tutoring, mentorship, or educational workshops and programming.
• Participation free for all first time tournament players
• Discounted fees applied to in house Jr. mixers and tournaments players.
• Tournament fees paid towards 1 off site tourament event of choose.
• Free travel expenses to any USTA sponsored quilified clinic or event.
• Elgible to receive any donated item given to the facilty/organizations at no cost.

 Application can be renewed every year.

More than 1 child: payment can be paid with one submisson or individually with each application.

Not interested in the yearly fee, no problem each event, activity, torunament, mixer, proshop item can be purchased individually. Prices will be announced for upcoming events. Always check the site (ace2deucecta.org or at the facility).
