Occupation
How did you hear about us?
How many pets do you have? List types and ages of pets.
Have you ever given up/lost a pet other than to death? If yes, please explain what happened.
Does your vet treat ferrets? Please list the name, address & telephone number of the veterinary hospital you use (or that has agreed to take you on as a client if this would be your first ferret).
Please list the name, address & telephone number of at least one emergency clinic you will use for the ferrets. This CANNOT be the same day practice you have listed above. It is a clinic that is open when your regular vet is closed - usually a 24/7 clinic for emergency/urgent care for evenings, overnight and weekend/holiday care.
Please provide the name/telephone/address of a veterinarian or vet clinic that you have used in the past for prior pets (not necessarily for ferrets).
Annual vet check ups are required, but plan on at least one major illness & one major emergency situation in a ferret's lifetime & usually middle of the night/weekend - what is your plan for handling medical emergencies & the potentially high cost of an emergency clinic?
Have you ever owned ferrets? If yes, for how long and how many do you have now?
Are your current ferrets ADV tested and vaccinated for distemper and rabies? If yes, when were they last tested/vaccinated?
How will your ferrets be housed? Cage or ferret safe area? How much time out of the cage will your ferrets have each day, on average?
What foods and treats (brand names, please) do you feed (or plan to feed) your ferrets?
Describe what a typical day would look like of your new ferret's life with you :)
Do you have an age preference? Male/female? Color?
What should we know about you while considering your application?
Please list any references (include mobile & email address) you have who could tell us what a wonderful ferret owner you would be? Prior veterinarians are a bonus.
