St. Ignatius Parish 2022 Christmas Eve Family Mass Children's Pageant Signup and Permission Form
Requirements:

• All parish children are eligible.

• There are THREE rehearsals (TWO mandatory), PLUS arriving early on Dec. 24.

• Permission form is required, by archdiocesan policy, to participate.

• Please submit forms online.

• Permission forms are due by Tuesday, December 13th, at 5:00 pm.

• Parents may wait in the Church during rehearsals if they wish.

Rehearsal Schedule:

Wednesday, December 14, 4:30-5:30 pm, in the Church Sanctuary (enter via Parker Avenue doors). Meet at the Communion Rail.

Sunday, December 18, 11:30 am-1:00 pm, in the Choir Room (enter via the Nursery--photo below). Costumes will be issued at this mandatory rehearsal.

Wednesday, December 21, 3:00-5:00 pm, in the Church Sanctuary (enter via Parker Avenue doors). Meet at the Communion Rail. Mandatory Dress Rehearsal!

Friday, December 24: Arrive by 3:00 pm, in the Church Sanctuary (enter via Parker Avenue doors). Meet at the Communion Rail.

Please dress warmly for all rehearsals and for the Mass as there is no heat in the Church.

With continued safety precautions during COVID-19, children are encouraged to wear masks during all rehearsals and during Mass.

I, {parentName}, the parent or guardian of the above named child, hereby, give my permission for her/his participation in the activity named above. I agree to direct my child to cooperate and conform to the directions and instructions of the parish, school or Archdiocesan personnel responsible for the activity. I agree, to the extent permitted by law, that in the event my child is injured as a result of her/his participation in the above named activity, including but not limited to transportation to and from the activity, whether or not caused by the negligence (active or passive) of St. Ignatius Church, the University of San Francisco, the Archdiocese of San Francisco, or any of their agents or employees, recourse for the payment of any resulting hospital, medical or related costs and expenses will first be had against any accident, hospital or medical insurance, or any available benefit plan of mine or of my spouse. I agree for myself and my child/children/ward to defend, hold harmless, indemnify, and release St. Ignatius Church, the University of San Francisco, the Archdiocese of San Francisco, or any of their agents or employees, from all claims or courses of action, which may result from such participation.
