Please fill out the following form in order to serve as a Mental Health Professional for an applicant seeking to participate in Miami's Leo Martin March of the Living for the 2023-24 school year.
The March of the Living is a six-month program culminating in a two-week international experiential program where teens from around the world come together each year and bear witness to the destruction of the Holocaust in Poland and then travel to Israel to rejoice in the Jewish Homeland. The program commemorates Yom HaShoah, Holocaust Memorial Day, marching from Auschwitz to Birkenau, and celebrates Yom HaAtzmaut, Israel's Independence Day, in the land of Israel.
The March can be physically and emotionally demanding. The content of the program - studying the Holocaust and visiting sites where many people died often brings up past traumatic experiences and losses. We want to make sure that the student you work with is capable of going through this experience, should they travel with us.
If you are receiving this form, the parents of the student have agreed to and signed the below consent to share information with the Miami Leo Martin March of the Living and CAJE (Center for Advancement of Jewish Education) Staff for the well-being of their child: I/we hereby consent for the Miami Leo Martin March of the Living program and CAJE staff to contact the below named mental health professional to obtain information about my teen, which is relevant to providing appropriate levels of care throughout the program and specifically during the travel component of the program while also determining if my child's mental state can manage the physically and emotionally demanding program. Additionally, if my teen exhibits psychological concerns before, during, or after travel, I authorize CAJE representatives to connect with my child's primary mental health professional to support their wellbeing.
