New Client Request Form
Hello! Thank you so much for being interested in booking with me and for taking the time to fill out my new client request form. There are a few things that you should know before booking with me: 1. I am only accepting new hair extension clients at the moment 2. All consultations are $25 due at time of booking 3. A deposit of $75 is required for hair extension services (see policy on website and below) 3. I am BEYOND excited to meet you and slay your hair!

When was your last color service and was it done in a professional salon or at home?

Please give me a detailed description on your color/chemical history. Example: What service you typically get done or any (artificial hair color / box dye in the last 3 years). Please be honest as this will help me keep your hair as healthy as possible through your coloring service.

Describe specifically what you are wanting done? (Ex: balayage created with extensions, caramel tones, all over black, full head of tape-ins, micro links 20-24 inches etc.)

*Please note that not every inspirational photo can be achieved in one session. It takes multi sessions in certain cases. Inspiration photos are ideas or goals. Not to be duplicated exactly, as to everyone’s hair is different. I will always try my best to get very close to what you want to achieve.

A $75 non refundable deposit is required to book/ hold any appointment. This amount will go towards your service at the end of your appointment if you do not want to prebook your next appointment. Deposits must be paid within 24 hours of the appointment being booked.

It is required to cancel/reschedule your appointment 48 hours before your scheduled appointment. If you reschedule in that time frame, your deposit will be transferred to your rescheduled appointment, if you cancel and don't reschedule, your deposit will NOT be refunded.
