As a part of Prime Time Staffing & Management LLC/Arise Virtual Solutions policies, you are required to pay a fee for your background check, training cost, security deposit and/or administration fee due to being an independent contractor and other reasons which you were explained. As stated, the fee ranges from $50.00-$110.00 depending on the company you choose. In addition, security deposit amount is determined by opportunity enrolled in.
This form is for individuals who already paid the above fee and would like to request a refund for that fee. However, our Payroll Department determines if an individual is eligibility due to varies of reasons which can include currently being unemployment, return of equipment and etc. If you are approved, then the Payroll Department will let you know which pay period you will receive refund and/or timeframe of refund. If you are NOT approved, then they will let you know the reason why.
The Payroll Department will reach out to you within 3-5 business days after submission of form regarding your request.
