Coaching Agreement

Dear Participant,

Welcome. During the coming months, you will learn ways to help yourself achieve a healthier diet and lifestyle. Please read the following and ask questions if there is anything unclear.

This Agreement is made today between Advanced Nutrition Solutions and the person named at the end of this document. The Program in which you are about to enroll in will include all of the following:

Scheduling

With this program being predominantly a web-based plan using email, text messaging, phone calls, and other support to provide you the best service imaginable. We will do weekly progress reports for all plans. I ask for my clients to weigh-in and update their weight in the 1st Phorm app by 10:00am on Friday. To weigh yourself properly you should do it upon waking, before food and water, and without clothes. Once an assessment is completed in the 1st Phorm app, you will receive feedback on your progress report. The client will also use the 1st Phorm App to track all foods. This also enters them to compete for $300,000 and allows Advanced Nutrition Solutions to make changes to plan weekly.

Payments and Refunds

There is no refunds on this program.

The Supreme plan does include a 30 day money-back guarantee if you follow the plan for 30 days and have no progress.

Confidentiality

The Coach will keep the Client's information private, and will not share the Client's information to any third party unless compelled to by law.

After the initial consultation, the coach will provide the following services:

1. Comprehensive meal plan and rationale behind it. 

2. Comprehensive workout plan that meets the client's goals.

3. Questions and Answers Throughout the Program

4. Access to 7 coaches, and a registered dietitian.

5. Weekly check-ins from coaches. If client fails to respond 3 weeks in a row, client will be moved to 1 time a month check in but can request weekly check ins again. 

6. Weekly changes, if needed, as long as client submits weekly assessment. All programming comes from Dustin Holston or our registered dietitian/doctor. 

It is the responsibility of the client to:

1. Complete weekly weigh-ins and Assessments

2. Track calories through the 1st Phorm app

3. Be honest about missing workouts or diet deficiencies

4. Ask questions when client doesn't understand

Dustin Holston is not a physician or psychologist, and the scope of his/her consultation services does not include treatment or diagnosis of specific illnesses or disorders. If you, the client, suspect you may have an ailment or illness that may require medical attention, then you are encouraged to consult with a licensed physician without delay. Only a licensed physician can prescribe drugs. Any mention of drugs during consultation is only for the purpose of providing a complete history of drugs that the client is taking and not for Advanced Nutrition Solutions to judge the appropriateness of the medication. Any change in prescription or dosage is a decision the client makes with his or her physician.

Advanced Nutrition Solutions is not a medical professional. The information I provide is based on personal experience, studies, and certifications including nutritionist, personal training, and a registered dietitian where applicable. Any recommendations ANS may make about weight training, nutrition, supplements or lifestyle, or information provided to you in person or through electronic communication should be discussed between you and your doctor.
