Our Policies:
Please return the Coaching Focus Form with payment prior to our session. This helps me to prepare for the coaching session so that you will receive the most benefit from the time.
Client services are confidential. Written authorization is required from you to release any information.
Your scheduled session is set aside for you. We do not double book appointments.
- Prepayment via Zelle, credit card or Paypal is required prior to each session.
- Please reschedule your session if you will be more than 15 minutes late, and notify me by e-mail or text if you are running late or have an emergency come up.
- If you must cancel your appointment and resechedule, a 24 notice would be appreciated.
Client Agreement: I understand that coaching is not therapy, and a coach does not diagnose illness, disease, any physical or mental disorder, nor do they prescribe medical treatment or pharmaceuticals.. I acknowledge that coaching is not a substitute for medical or psychological examination or diagnosis, and it is recommended that a qualified physician, licensed clinical social worker, counselor or therapist be seen for that service. It is my choice to engage a coach in order to help me achieve my stated goals, and understand that my participation in the coaching process is critical to successful outcomes. By my electronic signature below, I agree to the practice policy and client agreement above.
