    Employee Survey

    Welcome to the HCHC Diversity, Equity and Inclusion (DEI) Employee Survey.

    This survey is an important tool for all HCHC clinical staff and non-clinical staff members to complete. This DEI effort is designed to help increase employee competency in two areas:  SOGIE/LGBTQ and racial diversity.

    This survey should take approximately 20-25 minutes to complete and will help us:

    • Track our organizational progress toward increasing employee knowledge and skills in the two stated areas.
    • Determine HCHC's training needs to increase employee knowledge and skills working with SOGIE populations and diverse racial groups.

    Please note:
    While this survey focuses on two areas of diversity (SOGIE status and racial diversity), future surveys may cover additional areas of diversity such as age, physical ability, class background, etc.

    All of your answers will remain anonymous.

    As you complete the survey, try not to worry if you don't know the answers. This survey is not a test and you will not be scored for your responses to survey questions. If you find that you do not know an answer to a particular question, simply mark in the space provided that you do not know. You can also let us know if you'd like more information or training about a particular topic.

    We ask that you answer to the best of your ability and please do not look up answers to any of the survey questions.

    We are aiming for 100% participation from all HCHC employees so that accurate results from this survey can be used to inform our future DEI training needs and help us to continuously track our DEI progress.  

    This survey includes the following five sections:

  • Let's start with an explanation of DEI

    DEI = Diversity - Equity - Inclusion
  •  What is DEI?

     

    DIVERSITY – diverse organizations are made up of people from different racial, ethnic, socioeconomic and cultural backgrounds.

    EQUITY – equitable organizations recognize that employees have different cultural identities, life experiences, and access to opportunities and are responsive to these differences. 

    INCLUSION - inclusive organizations reflect the cultural diversity that exists in society and support the needs of people from diverse cultures, value their unique contributions, and encourage a sense of belonging for all employees.
